Geriatric
Practice
World Health Organisation (WHO) defines Active Ageing as the process of optimizing opportunities for health, participation and security in order to enhance quality of life as people age. Active ageing allows people to realize their potential for physical, social, and mental well-being throughout the life course and to participate in society.
Many older adults worry about their physical health and losing their memory. At The Geriatric Practice, we have a multidisciplinary group of Geriatrician (with sub-specialty interest in cognition), nurse and neuropsychologists to help evaluate memory/ cognitive difficulties. Not all memory difficulties are due to dementia. They may be a result of other medical illnesses, medications, mood and sleep disorders. Through a comprehensive evaluation, the team at The Geriatric Practice will help determine the cause of your cognitive difficulties and help allay any anxieties on you or your loved ones have about your memory.
While cognitive wellness tends to focus on healthy older adults, most of our recommendations pertain equally to people who are living with dementia and Alzheimer’s disease (the most common cause of dementia). Our team, together with clinical psychologists, occupational therapists, physiotherapists and speech therapists will work together with you and your loved ones, to educate you about your illnesses, map out individualized care plans to maximize engagement in life and quality of life.
At The Geriatric Practice, we bring together a group of passionate, like-minded healthcare professionals, with their vast experience in Geriatric and dementia care, to provide meaningful and evidence-based pharmacological and non-medical therapies to maintain cognitive function for as long as possible.
We are able to tailor individualised one-on-one cognitive stimulation therapy programmes, cognitive rehabilitation and cognitive behavioural therapy programmes, working together with occupational therapist and clinical psychologist to promote cognitive and mental wellness and brain health.
We are happy to offer you a specially curated dementia prevention program (please click on Caregiver Training and Prevention Program for more details)
